Male in need of community help (Westchester/)

Due to mental, phycial, cardio, and other health issues it has been a struggle these past couple of months.
I have tried everything but have been denied declined or turned away.
I know my list of needs of needs of needs is big but hopefully someone can help me with at least one thing from it or a couple of things.
As family is unable to help.
As people respond I will take off stuff from my list.
