Facility Information:
Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
Nursing caseload typically ranges from 6-15 residents
Our home sizes are small, including 6-bed and 15-bed facilities
High-acuity care is limited or prohibited by facility regulations
Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes

As a Registered Nurse (RN) you will be an integral part of our team, providing high-quality nursing care and support to individuals with disabilities in our Intermediate Care Facilities (ICF).
The Registered Nurse (RN) role offers you the opportunity to work in a small, close-knit community where you can build meaningful relationships with both the people we support and your fellow dedicated team members.
Registered Nurse (RN) Essential Responsibilities:
Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
Implement and oversee the health care plans for residents
Administer medications and perform medical procedures, as needed
Collaborate with a multi-disciplinary team to ensure the well-being of residents
Maintain accurate medical records and documentation
Offer support and guidance to: License Vocational Nurse (LVN), Direct Support Professionals (DSP)
Participate in complex case management and engage in continuous learning opportunities
Registered Nurse (RN) Qualifications:
Valid Registered Nurse (RN) license in the state of California
One (1) year experience as an RN; required
CPR certification
Strong communication and interpersonal skills
A genuine passion for providing quality care and support to individuals with disabilities
Ability to work effectively in a team-oriented environment
Willingness to adapt to the needs of the residents and facility
