The primary role of the Case Manager is to provide comprehensive strengths-based case management services to FTS clients including the successful transition into and maintenance of permanent housing.
* Monitor and conduct weekly case management meetings with clients on a one-on-one basis making any necessary recommendations for additional services
* Monitor client files to ensure quality, grant compliance, and all information is accurate and up to date
* Maintain client confidentiality in accordance with FTS policy and procedures as well as funding regulations
* Aid individuals and/or families struggling with permanent housing, social services, mental health, medical concerns, domestic violence, and parenting assistance (as needed)
* Locate and make appropriate recommendations for community resources and linkage to services for clients
* Evaluate client’s readiness to transition; including the formulation of service plans and goals, and aid clients in implementing service plans
* Attend Multidisciplinary Case Planning and Supervision Meetings
* Maintain working knowledge of all FTS policy and procedures
* Perform duties as assigned by supervisor and/or Directors
