SUMMARY
The Community Schools Case Manager provides individual/family services, case management, and consultation services for a caseload of children and families within various schools and communities (home, office, etc.
) The position coordinates services at the school site and supports colleagues in including students successfully in diverse settings by providing groups, resources and development opportunities.
Works collaboratively with CS coordinators, classroom teachers, school leadership and extended school team to maintain the academic, social and emotional needs of each student and family.
ESSENTIAL DUTIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Provides skill building to clients on a broad variety of areas of focus, linkages, and support services.
Ensure compliance with DMH billing requirements and activities provided, within CII’s documentation standards.
Serves as a member of a multi-disciplinary team to implement academic, behavioral, and therapeutic services for students in need of intervention.
Coordinates the collection and input of evaluation data used to measure ongoing program effectiveness of services and program outcome measures.
Facilitates home visits to support family and student linkages and connections to resources.
Implements research-based interventions aligned with the three tiers of MTSS, including individual, group, and family services.
Documents interventions and services provided and provide necessary reports on student progress.
Provides consultation to school staff on safety assessments, mental health crises, and accessing social service.
Participates in regular supervision to enhance practice.
Establishes and maintains rapport and effective working relationships with children and families and works cooperatively within CII, schools and community resources.

SUPERVISORY RESPONSBILITIES This job has no supervisory responsibilities EDUCATION & EXPERIENCE 1 year of community-based direct service experience.
Bachelor’s degree in education, social work, public health, or related field OTHER QUALIFICATIONS Valid driver's license and state-required auto insurance.
Required to travel extensively (> 50% of the time).
Spanish / English bilingual required.
Ability to handle confidential information.
Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
Sensitivity to service population’s cultural and socioeconomic characteristics.
Excellent verbal and written communication skills.
Free from alcohol and drug abuse.
Understands CII’s basic philosophy and participates fully in carrying out its mission.
