We develop, own and manage multi-family supportive housing and licensed residential care homes.
Additionally, we implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services and health care sectors.

PROGRAM SUMMARY The Case Management Department at Brilliant Corners includes both Permanent Supportive Housing and Rapid Rehousing programs and is supported by Housing for Health (HFH) at the Los Angeles County Department of Health Services (DHS).
The Permanent Supportive Housing program was designed to serve individuals experiencing homelessness who have a chronic illness or physical disability, and/or are high utilizers of DHS services.
The goal is to transition individuals out of homelessness and into permanent housing, in order to obtain stability in all areas of their lives.
POSITION SUMMARY The Permanent Supportive Housing Senior Case Manager will provide intensive case management services to a caseload of 15-20 program participants.
The Senior Case Manager will assist clients to reach their identified goals and connect clients to community resources and services.
The Senior Case Manager will work collaboratively with the Permanent Supportive housing and employment teams to ensure that clients reach the ultimate goal of self-sufficiency.
Additionally, the Senior Case Manager will help facilitate the training and integration of new staff members and interns.
The Senior Case Manager will also assist with administrative duties to enhance the overall efficiency of the program, including data entry and conducting eligibility meetings.
RESPONSIBILITIES Process and accept referrals from partnering agencies Conduct outreach and engagement in the community in order to locate referrals once they have been assigned by DHS.
Conduct assessments and re-assessments, including a comprehensive psychosocial assessment, in order to determine clients’ eligibility for the program and evaluate clients’ needs and strengths.
Develop and implement individualized case management services plans that ensure housing stability and identify short and long-term goals.
Provide support throughout the housing process including but not limited to temporary housing placement, unit identification, application submissions, lease signing, and ongoing housing retention.
Provide education to clients on tenant rights and responsibilities, eviction prevention counseling and advocacy.
Assist with data entry and administrative tasks, including creating CHAMP applications for newly enrolled clients Help facilitate the training and integration of new staff members, including reviewing processes, arranging shadowing opportunities, and being a point person for questions.
Assist with the creation of a training manual, resource sheets, and other program material as needed.
Attend designated community meetings and events.
Support clients to access and retain housing, including educating clients on tenant rights and responsibilities and providing eviction prevention counseling and advocacy.
Develop and implement, in collaboration with clients, individualized case management services plans that identify short and long-term goals.
Meet with clients frequently and regularly in the home, community, and office.
Provide crisis intervention as needed.
Provide linkage to community resources, benefits, and services which may include physical and mental health, substance use treatment, education/vocational programs, employment services, health insurance, disability benefits, and other sources of financial assistance.
Provide life skills education and assistance, including budgeting, apartment upkeep, stress management, conflict resolution, etc.
Build and maintain partnerships with community organizations, service providers, public entities, and landlords to ensure that clients have access to comprehensive services.
Maintain updated and accurate client files and documentation.
Participate in case conferences, staff meetings and other agency events as requested MINDSETS Good customer service matters to you.

Requirements PROFESSIONAL EXPERIENCE At least 2 years of experience providing direct mental health and/or intensive case management services to vulnerable populations Experience working with individuals with chronic medical and/or behavioral health conditions, ideally individuals residing in permanent supportive housing.
KNOWLEDGE, SKILLS, AND ABILITY · Strong interpersonal and client engagement skills · Excellent verbal and written communication skills · Flexibility regarding scheduling and prioritizing of tasks · High degree of attention to detail and the ability to manage multiple deadlines efficiently and simultaneously.
· Willingness to travel and spend significant time in the field · Basic computer knowledge (MS Word and Excel required) · Ability to work independently and utilize critical thinking skills in decision-making situations · Good organizational and record-keeping skills · Possession of a valid, clean California driver’s license and a personal insured vehicle are required Travel Requirements This position requires frequent visits to properties, attendance to on-site vendor meetings, community meetings, funding workshops, trainings, and other events primarily in the Los Angeles Area.
Opportunities to attend events outside Los Angeles area, including Brilliant Corners’ sponsored events throughout California, or nationally may also be presented for this position.
If driving a personal or company vehicle a valid, clean CA driver’s license and personal auto insurance may be required.
Salary range for this position is $27.
