Hospice Master of Social Work (MSW)

is seeking for a Master of Social Work (MSW).
Under the supervision of Registered Nurse, the Master of Social Work is responsible in assessing patients, monitors planned actions, and other functions and responsibilities.
*ESSENTIAL DUTIES AND RESPONSIBILITIES:* * Determines nature of patient’s situation by interviewing patient, assessing medical, psychological, emotional and social information * Establishes course of action by exploring options and setting goals with patient.
* Interpreting attitudes and patterns of behavior.
* Maintains records of case by documenting patient’s situation and patient’s actions.
* Maintains operations by following policies and procedures, participating in quality reviews and reporting needed changes.
* Contributes to team effort by accomplishing related results as needed.
*Bilingual:* Spanish and/or Mandarin is a plus *Requirements for Position:* * Master’s in social work accredited by the Council of Social Work Education * Licensed clinical social worker preferred * At least 1-year experience in either of the following: o Hospice o Home Health o Hospital o Subacute * Valid California Driver's License * Reliable Transportation * Proof of auto insurance *Job Types:* Full-time, Part-time Benefits: * Dental insurance * Health insurance * Paid time off * Retirement plan Job Types: Full-time, Part-time Benefits: * Dental insurance * Health insurance * Paid time off * Retirement plan Schedule: * 8 hour shift * Monday to Friday Experience: * Social work: 1 year (Preferred) * Hospice: 1 year (Preferred) Language: * Mandarin/Spanish (Preferred) Work Location: On the road
