Examples of Essential Duties:
Evaluate patients’ psycho-social needs;
Document history, care plans and prepare reports as needed;
Provide clinical and administrative support to case managers;
Assist with crisis management;
Ensure proper documentation and charting is completed in a timely and accurate manner;
Hire and manage staffing of intensive case management team.
Experience Requirements:
2 years of experience working with homeless adults or those living with mental illness and/or substance abuse disorders
Knowledge of chemical dependency and substance abuse treatment, is preferred
Education Requirement:
Graduation from an approved Masters Program in Social Work
Qualifications:
Current California Certification as an Associate Clinical Social Worker (ACSW)
Current AHA-approved CPR card
Valid Drivers License
Personal Vehicle
