The PGY-2 Pharmacist Transitions of Care Resident at Cedars-Sinai Medical Center in Los Angeles, California is responsible for participation in and coordination of interdisciplinary transitions of care (TOC) initiatives.
The employee performs job duties with sensitivity to the patient’s diverse cultural background.
• Emergency Department Medication Reconciliation
• Collaboration with Hospitalist Programs
• General internal Medicine Transitions of Care initiatives
• Participation in Research and Outcomes Assessment
• Precepting PGY1 Residents and Students
• Training of medication reconciliation technicians
• Participation in Medical Staff Committees and Interdisciplinary Committees
• Participation in Patient education activities
Educational Requirements:
Graduate of an accredited School of Pharmacy with a Doctor of Pharmacy degree.
Licenses:
Current intern license in California
California pharmacist licensure obtained within 90 days of program initiation
BLS certification as outlined in the Pharmacist Responsibilities in Cardiopulmonary Resuscitation(C.
P.
R.
) and Code Blue Procedure: Pharmacy Policy and Procedure
Experience:
Candidates must have completed a PGY1 residency or have a minimum of 2 years of hospital experience.

Sufficient mobility to access equipment, patients and other customers.
