Overview:
As a Licensed Vocational Nurse (LVN) in Ambulatory Care, you will be an essential member of the healthcare team, providing nursing care in outpatient settings such as clinics, medical offices, and ambulatory care centers.
Your role involves assisting healthcare providers, delivering patient care, and ensuring the smooth flow of operations in the ambulatory care environment.
Key Responsibilities:
Patient Assessment and Triage:
Conduct initial patient assessments, including vital signs, medical history review, and chief complaint evaluation.
Triage patients based on acuity and collaborate with healthcare providers to determine appropriate care plans.
Patient Care and Procedures:
Administer prescribed medications and treatments under the direction of healthcare providers.
Perform various nursing procedures, including wound care, injections, and diagnostic tests.
Assist with minor surgical procedures and other outpatient interventions.
Patient Education:
Provide patients with information on their conditions, medications, and treatment plans.
Offer health education and lifestyle counseling to promote wellness and prevent illness.
Documentation and Record Keeping:
Maintain accurate and up-to-date patient records, documenting assessments, interventions, and patient responses.
Ensure compliance with legal and regulatory standards for record-keeping and documentation.
Collaboration with Healthcare Team:
Collaborate with physicians, nurse practitioners, and other healthcare professionals to coordinate patient care.
Communicate effectively with other team members to ensure a cohesive and patient-centered approach.
Medication Management:
Administer medications as prescribed, ensuring accuracy in dosage and patient understanding.
Educate patients on medication management, potential side effects, and the importance of adherence.
Patient Advocacy:
Advocate for patient needs and preferences, addressing any concerns or barriers to care.
Collaborate with patients in the development of care plans and treatment goals.
Medical Equipment and Supplies:
Ensure the availability and proper functioning of medical equipment and supplies needed for patient care.
Assist in maintaining an organized and clean work environment.
Qualifications:
Current licensure as a Licensed Vocational Nurse (LVN) in the state of CA.
CPR and Basic Life Support (BLS) certification.
Experience in ambulatory care settings is preferred.
Proficient in basic nursing skills, including venipuncture, IV therapy, and medication administration.
Strong communication and interpersonal skills.
Physical Requirements:
Ability to stand for extended periods and move swiftly within the ambulatory care setting.
Lifting and positioning patients as needed.
Fine motor skills for performing nursing procedures.
Working Conditions:
Primarily within the ambulatory care setting, which may include clinics, medical offices, and specialty outpatient centers.
Regular business hours, with the potential for occasional evenings or weekends based on the needs of the healthcare facility.
