00]* Are you ready to contribute to a team, committed to excellent customer service and dedicated to each individual’s unique talent? *If yes, how you will contribute to the team:* As a *Case Manager LVN/LPN (Floater)* you will oversee and help manage the delivery of care from pre-admission to post-admission.
You will help ensure residents and their families receive appropriate utilization of services and coordination of care.
*Must have:*
* Successfully graduated from an approved and certified LVN program
* Current and unencumbered license to practice as an LVN in the state
* Ability to make quick judgements and work independently
* Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
*Nice to have:*
* 2 years’ experience as an LVN/LPN
* Active patient or bedside care within the past three years
