If yes, how you will contribute to the team:
As a Case Manager LVN/LPN you will oversee and help manage the delivery of care from pre-admission to post-admission.
You will help ensure residents and their families receive appropriate utilization of services and coordination of care.
Must have:
Successfully graduated from an approved and certified LVN program
Current and unencumbered license to practice as an LVN in the state
Ability to make quick judgements and work independently
Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
Nice to have:
2 years’ experience as an LVN/LPN
Active patient or bedside care within the past three years
