*Position Overview* This position is responsible for coordinating a variety of professional and administrative care activities for the On-site Clinic located at The Center.
Responsibilities range from coordinating registration form intakes, answering complex programmatic questions, directing clients, troubleshooting routine to complex issues, performing advanced navigation and coordination through the healthcare continuum, to making transportation arrangements for clients.
*Essential Functions* * Collaborates with Saban Community Clinic and other health, mental health, and substance use treatment providers to develop robust on-site wellness supports for Center participants.
* Coordinates a variety of clinic activities which includes coordinating intake, answering complex programmatic questions, directing clients, troubleshooting routine to complex issues, performing advanced navigation through the healthcare continuum, and making transportation arrangements.
* Connects clinic participants to internal and external case management and supportive services and regularly communicate with assigned case managers.
* Greets and assists participants in scheduling and checking in for appointments, and escorts them to the clinic area.
* Assists clients in completing initial intake paperwork and reviews paperwork for accuracy.
* Assists participants in navigating the healthcare continuum including on-site medical and behavioral health appointments, connection to insurance benefits, and facilitation of off-site specialty appointments.
* Assists clients in arranging transportation to manage their healthcare which may include picking up medications, lab work appointments, specialty referrals, and other services not provided on-site at The Center.
* Transports and escorts clients on an as-needed basis.
* Helps clients in distress by responding to and alerting appropriate staff of emergencies.
* Ensures program expectations, as they are related to The Center, are being met and assists in troubleshooting any issues with Center staff and stakeholders before escalating issues to the appropriate officer.
* Responds to phone calls and e-mails in a timely manner to schedule and confirm appointments, coordinate care for participants, and troubleshoot barriers to access.
* Develops and maintains an understanding of eligibility and insurance criteria for various programs.
* Coordinates with other internal staff and external stakeholders to evaluate operations and efficiency improvements in how clinic services are delivered.
* Works to optimize participant satisfaction and usage of internal and external resources (provider time, exam rooms) by ensuring that available clinic appointments are filled in an expeditious manner.
* Educates and supports participants by explaining clinic services, processes, and providing guidance on how appointments will function in order to soothe any anxieties they may have.
* Maintains business office inventory and equipment by checking stock to determine inventory level, anticipating needed supplies, and placing and expediting orders for supplies.
* Protects participants’ rights by maintaining confidentiality of medical, personal, and financial information.
Follows HIPAA guidelines to maintain records of client services and accounts.
* Maintains operations by following Center policies and procedures and reporting needed changes.
* Coordinates with other staff to define relevant program data points, develop workflows to track monthly outcomes, and report outcomes out to Center leadership.
*Other Functions* * Attends events or assigns staff to attend events as required.
* Attends seminars, conferences and trainings for professional development as required.
* Performs all other related duties as requested.
*Education & Experience* *Clinical Master’s Degree such as an MSW or MFT is required for this position.
* We seek applicants with a minimum two years of experience working in a fast paced, customer-centric environment and working with diverse and vulnerable populations.
Experience must include coordinating activities with and for clients.
Experience working in a medical office environment is highly preferred.
Any equivalent combination of education, experience, or training that has prepared the incumbent to perform the essential duties of the position.
*Knowledge, Skills and Abilities* The position requires the following knowledge: * General knowledge of general office practices.
* General knowledge of various computer software including MS Word, Excel and Outlook.
* General knowledge of the healthcare continuum.
* General knowledge of medical insurance in the State of California.
The position requires the following skills: * Proficient in computer applications, such as work processing, spreadsheets and internet usage to conduct research.
* Judgment and decision-making skills with the ability to identify critical information and choose effective course of actions.
* Organizational and planning skills.
* Strong interpersonal and conversational skills, that elicits relationship building.
* Strong record keeping skills to track metrics and participation.
The position requires the ability to: * Demonstrated ability to conduct routine to complex research in the areas of medical insurance and the healthcare continuum.
* Demonstrated ability to conduct one’s self in a calm and professional demeanor when dealing with the public and/or with difficult situations.
* Communicate appropriately both verbally and in writing to a variety of internal and external stakeholders.
* Demonstrated ability to work independently and possess strong multitasking and problem-solving skills.
* Provide customer service to internal and external stakeholders.
* Positively engage staff in the resolution of volunteer and relationship issues.
* Work in a highly collaborative team environment.
* Understand, demonstrate and promote the Center’s values and model those values in the performance of all duties.
*Licenses & Certifications* CA driver’s license or equivalent.
This position is eligible for clinical supervision for applicants pursuing licensing in the state of California.
Applicants must register with the appropriate regulatory agency for licensing within 3 months of appointment.
*Special Requirements* Successfully pass a background check.
*Working Conditions* Must be able to lift 20 pounds on a frequent basis.
Work both indoor and outdoors in a variety of weather conditions.
Work in an office building setting with a moderate noise level.
