As a Case Manager/Social worker your duties will range from completing documentation, implementing treatment plans, and facilitating quarterly meetings.
Job Responsibilities:
Weekly updates of county control sheets/ Coordinating weekly rounds meetings.
Scheduling of Quarterly Meetings and completing formal quarterly reports of patient’s status.
Developing Patient Care Plans on admission, quarterly, and significant changes in status.
Supervision of a case load of 55+ residents
Collaborative Interfacing with other Departments
Regular chart audits to ensure monthly progress notes are up to date.
Assisting counselors, when needed, in dealing with patient needs.

Crisis intervention and management.
Completion of HS231 forms as required by regulations
Any other duties as assigned by the Program Director or Assistant Program Director.
Qualifications / Essential Skills:
Follow facility policy as it relates to patient care, treatment, intervention and documentation.
Sufficient verbal and written communication skills to perform task required.
Supervisory experience recommended
Must uphold and maintain professional behavior at all times.
Strong problem solving and critical thinking skills.
Dependable and Good work ethics.
Ability to interact with residents in the milieu and model appropriate social and professional behavior.
Treat all residents with dignity and respect.
Experience in mental health and substance abuse populations preferred but not required.
Ability to work with patients in a manner that is productive and coincides with patient treatment goals.
Education
Bachelors in Social Work, Psychology, or similar field
ADMISSIONS:
As the Admissions Coordinator, you will work to attract suitable candidates for admission and integrates them seamlessly into the system.
You will establish and maintain a constant working relationship with all potential referral sources, conduct tours and pre-admission interviews with prospective residents/responsible parties and explain all facility services.
Our Admission Coordinator receives and reviews all inquiries regarding facility admissions via telephone, email, fax, and in person.
Extracts information from referral and completes information sheet for team review.
He/she will also review admissions for suitability with the Administrator and the Wellness nurse, as appropriate, for an admissions decision.
You will also be responsible for following up with the resident after they have been admitted to ensure their transition is smooth and they are satisifed with the services being rendered.
EMPLOYMENT STANDARDS
Knowledge:
Working knowledge of common medical supplies and equipment, and their proper methods of storage;
Working knowledge of the methods and procedures involved in the receipt, storage and issuance of supplies and equipment;
Working knowledge of methods and procedures involved in the maintenance of accurate inventory records.
Ability to:
Make arithmetic computation;
Operate a large supply room containing a variety of medical and other supplies and equipment.
Prepare and maintain records of requisition;
Read, understand and maintain records of purchase orders and storeroom operations;
Plan and efficiently arrange the storage of a variety of supplies and equipment;
Maintain perpetual inventory records to insure adequate stock;
Safely lift and move heavy and bulky objects.
Training and Experience:
One year of experience which would demonstrate application or possession of the knowledge and abilities.
