As a key member of our care team, the ACO LVN will be responsible for providing care and management to a panel of home -based patients.
This position also participates in patient engagement, post-discharge visits, patient phone calls and facilitation of tele-visits by clinicians.
Dedicated to improving the health and well-being of patients, the position collaborates with the care team as well as PCP to discuss immediate and ongoing patient care needs.
Key Duties and Responsibilities Provides direct patient care as defined in the State Nurse Practice Act.
Implements current nursing practice following a comprehensive assessment and the plan of care.
Provide accurate and timely documentation of patient services to reflect the plan of care.
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
Participates in coordination of palliative care services, appropriately reporting the identified needs to the interdisciplinary group.
Uses equipment and supplies effectively and efficiently.
Provides appropriate pain/symptom management.
Evaluates patient’s response to treatments/medications.
Participates in personal, professional growth and development.
Also participates in quality assessment performance improvement teams and activities.
Performs other duties as assigned by the Nurse Practitioner and MD.
Education and Experience Currently licensed as Vocational Nurse in the state of California One (1) to three (3) years of nursing experience.
Community health/hospice or medical/surgical and palliative care experience is preferred.
Possesses and maintains current CPR certification as required by organizational policy.

Advanced knowledge of word processing, graphic presentation and computer software related to specific tasks.
Demonstrated excellent computer and word processing skills with special emphasis on calendaring, presentation, and spreadsheet capabilities.
Working knowledge of company policies, procedures, and operations Excellent composition, grammar, and business language skills Excellent communication and interpersonal skills with the ability to effectively communicate with all levels of management, patients, and family members.
Creative, flexible, well organized, resourceful, and detail-oriented Excellent judgment in handling confidential and sensitive information Ability to work independently, set priorities and handle multiple tasks with a high level of efficiency.
Establishing and maintaining cooperative working relationships with others Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Ability to work across locations and time zones.
