*Summary:* Under the direction of the Prevention Education Manager, the *Community Health Worker* is a direct service position that is responsible for supporting efforts to increase access to and utilization of Substance Use Disorder (SUD) services in Los Angeles’ Koreatown and neighboring communities.
The Prevention Specialist will plan, implement, and evaluate activities that will help build capacity and expand access to improve quality SUD services through evidence-based, culturally competent, trauma-informed, and age appropriate methods.
*This project is contracted to end June 30, 2025.
* Performance is reviewed on a continuous basis with specific goals and objectives identified throughout the year.
This is a full-time, non-exempt position, working 40 hours a week.
Typical schedule is 8:30 am to 5:30 pm with a one-hour lunch but can vary based on program and community needs.
*This position is hybrid and some weekend shifts are required*.
Full COVID-19 vaccination is required.
*Duties / Responsibilities:* * *Provide program facilitation and implementation by: * * Working with local providers to strengthen SUD treatment referral linkages and offer navigation services.
* Using a family-based intervention, engage parents and family members to help their loved ones seek help.
* Recruiting and facilitating youth volunteers program to provide youth and young adults a platform to connect, build support, and to voice their input on policies and service design and implementation.
* Establishing and engaging an ongoing working relationship with community-based organizations, clinics, schools, churches, parents and youth.
* Developing and conducting SUD prevention and treatment outreach materials and educational workshops for community partners.
* Establishing and maintaining referral connections alongside KYCC Recovery Services unit staff.
* Support Koreatown Storytelling Program with storytelling efforts including participant recruitment and conducting oral history interviews.
* Developing outreach plan that will include dissemination of information and referral for counseling services, overdose prevention resources and Naloxone and fentanyl strips distribution, referral to MAT services.
* Supporting with event planning and implementation.
* Distributing fentanyl strips and naloxone.
* Supporting with a multimedia campaign aimed at educating the general community on SUD services and increasing access to services through multimedia creation and development, and data collection.
* Supporting oversight of all contract compliance activities regarding relevant programs including, but not limited to: meeting contract objectives, documentation requirements, tracking of outcome measures, and coordinating evaluation activities.
* Assisting in monitoring program for all relevant programs to ensure satisfactory compliance with internal agency procedures and contract requirements.
* Ensuring that strategies are culturally appropriate.
* *Attending relevant meetings and trainings.
* * *Other related duties as assigned by the Prevention Education Manager.
* *Minimum Requirements / Qualifications (All applicants MUST meet the minimum qualifications):* * A Bachelor’s degree from an accredited college or university in public health, public policy and/or social sciences AND one (1) year of experience in the fields of public health, public policy, community engagement and/or related field.
–OR– Three (3) to five (5) years full-time experience in the fields of public health, public policy, community engagement and/or related field.
* *Bilingual capacity (Spanish/English)* * Knowledge and understanding of SUD prevention and treatment.
* Possess flexibility, initiative and ability to work under pressure.
* Strong interpersonal, organizational, written and communication skills with colleagues, clients and representatives from other community organizations.
* Strong commitment and competency to work with a multi-cultural community.
* Ability to work collaboratively with others and a willingness to participate fully in a team process.
* Self-directing and ability to work independently as required.
* Ability to organize and prepare documentation in a timely manner.
* Demonstrable working knowledge of the use and operation of personal computers.
* Proof of passing a TB test and fingerprinting clearance (Live Scan).
* A valid California driver’s license is required.
* Proof of eligibility to work in the United States.
* Physical requirements: Seeing to inspect written documents; ability to communicate with KYCC staff, clients and public; sitting, standing and walking for extended periods of time; bending, kneeling and reaching to retrieve and replace files; and dexterity of hands and fingers to operate office equipment; and ability to lift up to 40 lbs.
* Certification in CPR and First-Aid training.
