org Position Summary: The Residential Case Manager is responsible for the orientation process and case management of 10 pregnant residents at Harvest Home’s residential home in the Venice Beach neighborhood.
Weekly Schedule / Availability: Full-time, 40 hours per week with flexibility and availability in hours and days to work nights and/or weekends, as needed.
Primary Responsibilities: Coordinate resident move-ins, orientations, and move outs Provide individual case management for 10 residents (adult pregnant women ages 18+) Meet with residents at least weekly to develop a case plan, assist in overcoming barriers, and link residents to necessary resources Assist residents in maintaining and increasing government assistance and supportive services, as necessary.
Assist residents in obtaining identification (e.
g.
, driver's license, birth certificate, etc.
) Develop transition plan for residents, alternative housing plans and/or link resident to appropriate housing programs and resources when resident moves out of the home Collaborate with the multi-disciplinary Care Coordination team regarding resident strengths, needs, and standing in the program Facilitate linkage to mental health, substance abuse, and medical services, and provide transportation when needed Input, maintain, and organize detailed, accurate, high-quality documentation in resident files and database Advocate on residents’ behalf for support services and support residents in decision-making and self-advocacy Provide and/or coordinate crisis intervention assistance Promote personal growth, independence, responsibility, and accountability among the residents in the housing program and inspire them to reach their full potential Collaborate with the Residential Life Coordinators to ensure the residents’ optimal functioning in the residential program including upholding household requirements, establishing a system of accountability, and follow through on areas of change, modification, and/or engagement Initiate, institute and keep residents accountable for improvement plans where necessary/appropriate Develop and maintain working relationships with local service providers, outreach workers, case managers, and collaborative agencies Engage with external organizations to keep abreast of new, changing, and/or additional opportunities to ensure case management services and resources are appropriate, relevant, up-to-date, and applied to resident and program needs Build rapport and respect with residents as foundation for professional relationships.
Utilize a variety of approaches as appropriate for care, guidance, intervention, and/or service delivery such as Trauma-Informed Care, Resident-Centered, Solution-Focused, and an empowering strengths-based approach Listen attentively to resident concerns and suggestions and address their issues fairly and professionally, coordinate with immediate supervisor for supervision Model Harvest Home’s cultural values in communication, behavior, and professionalism Attend all staff and program meetings, training, and events Other duties as assigned Qualities and Skills Ability to work well both independently and collaboratively as a member of a multi-disciplinary service team Skilled in Trauma-Informed Care, Resident-Centered, Solution-Focused, and an empowering strengths-based approach High level of self-accountability and ethical standards Resourceful, motivated self-starter with the ability to manage time well and work autonomously; to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with program, documentation, and organization responsibilities Excellent critical thinking skills, solution-focused approaches, and crisis intervention skills with ability to exercise and model good judgment Excellent communication skills Strong relational skills with the ability to build rapport and trust quickly Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) required Proficiency in Project Management tools (Asana, Outlook calendar) Ability to set, model, and uphold appropriate personal and professional boundaries and relationships with staff, residents, volunteers, and community providers Education and Experience BSW with 2+ years relevant experience; MSW; or other equivalent experience Strong understanding of and experience with the complexity of poverty and women’s homelessness, mental illness, trauma, substance abuse, health issues, permanent supportive housing model and evidence-based practices Experience working with many of the following: high-risk populations, women, homelessness, pregnancy, infancy, victims of trauma, mental health, substance use, domestic violence, poverty, TAY, Foster care, DCFS, etc.
in social welfare, social work, counseling, hospitality, and related fields Experience working with diverse, high-risk populations.
Preference given to candidates with human services experience in LA County and with knowledge of local agencies and organizations.
Preference given to candidates with knowledge of or experience with infant/child development.
