86 Per Hour SUMMARY The HFH Enriched Residential Care (ERC) program places homeless individuals with functional deficits into licensed adult residential facilities (ARFs) and residential care facilities for the elderly (RCFEs) and then provides technical assistance/quality assurance services to facility administrators to ensure that HFH clients receive high quality care and attention so that their health, quality of life, and community integration are optimized.
The Clinical Social worker as part of the Client Wellness Team, will work under the supervision of the Social Work Supervisor in the ERC program.
The senior clinical social worker will provide behavioral health services and case management services to patients who are placed in the ERC program.
The focus will be to assess and when needed treat the complicated psychosocial problems of ERC clients including but not limited to medical condition and/or functional status, untreated or under- treated mental health or substance abuse condition.
The Social workers primary responsibility is to provide comprehensive assessment of the behavioral and mental health needs of our high acuity ERC patients.
Providing full assessment of their situations, strengths, and support networks of clients at the ERC facility.
The goal is to provide assessment, clinical guidance and consultation, and development of a comprehensive care plan that addresses the psychosocial, mental health and psychiatric needs of the clients.
The care plans who will be executed by the medical case works should include the involvement of facility staff and support to provide quality care and support to the patients.
The clinical social worker will be working closely with the medical case workers, nursing managers, and program managers to facilitate the transition of homeless individuals into permanent housing and provide comprehensive care within a 24-hour licensed care facility.
This includes working closely with staff to promote care coordination, care integration and collaboration with facility operators and other treatment team members to provide high quality care to our ERC patients.
ESSENTIAL FUNCTIONS Assesses and treats the complicated psychosocial problems of ERC patients including but not limited to medical condition and/or functional status, untreated or under- treated mental health or substance abuse condition, economic instability, legal problems, and inadequate social supports, housing and transportation.
Work closely with the medical case workers for both low and high acuity patients and in partnership with the medical team and program managers, will assess, develop, and help implement care plans for the patients in the ERC program.
Provides consultation to other multidisciplinary team members to support staff in delivery of care support management of chronic medical and behavioral condition.
Help ensure high quality care by providing comprehensive assessment, conferring with program managers/nursing to help identity most appropriate placement options and most appropriate treatment plan to address the patient’s mental health, Substance abuse and Behavioral health needs.
Conducts comprehensive behavioral health assessments and care planning of ERC patients and develops treatment plans that are sensitive to the patient’s diverse needs and abilities.
Provides direct service and consultation to others providing services to the patients, using motivational interviewing and a trauma informed approach.
Respond to incident reports, as needed.
Create or update client care plan identifying wrap around supports that align with assessment.
Participate in case conferencing with the care team, as needed.
Coordinate SUD and/or mental health services on behalf of the client.
Provide technical assistance and training to facility operators to better serve clients in ERC.
Upon referral of certain high risk and complex patients, the social worker will provide direct services using behavioral health techniques including, but not limited to, motivational interviewing, cognitive behavioral therapy, relapse prevention therapy, psychotherapeutic and/or systems interventions, and trauma-informed care, delivered in an individual and/or group setting.
ADDITIONAL ADMINISTRATIVE FUNCTIONS Participates in team huddles, case conferences, and multidisciplinary team meetings as needed.
Participates in performance/quality improvement (PI/QI) activities and contributes to policy development as needed.

Four years or more of professional social work experience post graduate school, two of which functioning in an independent capacity providing highly specialized social work services.
CERTIFICATES/LICENSES/CLEARANCES An active and valid license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences.
The required license MUST be current and unrestricted; conditional, provisional, probationary, or a restricted license will not be accepted.
A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.
