Since 1976, St.
Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice and throughout Los Angeles County.
The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through 20+ programs in four pillars of service: Outreach and Engagement, Housing Services, Mental Health, and Education and Vocational Training.

Joseph Center serves more than 10,000 people each year.
Brief Description of Principal Activities Multifaceted intervention, prevention, mental health, and housing assistance is offered through street outreach and 9 service sites throughout Los Angeles County.
St.
Joseph Center also provides on-site services at five permanent supportive housing developments, and will provide services at an additional six more sites in the coming year.

Joseph Center’s total holistic approach to services provide our clients with the help they need to regain their stability.
Job Description Under the supervision of the Program Manager of the Homeless Service Center (HSC), the Health Liaison Improves physical health of people experiencing homelessness, increases linkages to housing and other social services and provides a safe place for clients to recover from treatment.
Principle Responsibilities: Collaborates with PCP Providers, and community homeless service providers to provide enhanced care coordination for eligible patients and/or entities to ensure a seamless experience for the patient.
Conducts case management services to include intake and assessment, develop comprehensive care plan, implementation of care plan, subspecialty referrals, monitoring, and follow-up.
Advocate on behalf of Members with health care professionals Use motivational interviewing, trauma- informed care, and harm-reduction approaches Collects and enters necessary data for reporting purposes, completes and submits monthly project activity and other reports as assigned.
Participates in weekly case conferences with physicians and peers to assist in problem solving related to clients and to ensure that professional guidance and high-quality case management services are being provided.
ESSENTIAL DUTIES & RESPONSIBILITIES The Health Liaison ’s key areas of responsibility include: Key Areas of Responsibility & Essential Duties: Assess homeless individuals utilizing the Vulnerability Index (VI) SPDAT.
Link clients to physical health, mental health, and other supportive services.
Develop housing plans with clients, link clients to appropriate permanent housing programs.
Assist clients to enroll in mainstream benefits and obtain identification (e.
g.
, California ID, birth certificate, social security card etc.
) Document services for each client and enter appropriate data into the region’s Homeless Management Information System (HMIS) Other duties and responsibilities may be assigned.

Qualifications QUALIFICATIONS Knowledge, Skills & Abilities: Must be highly motivated and a self-starter.
The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.
Must have excellent administrative and organizational skills.
Strong knowledge of homeless services is preferred.
Bi-lingual Spanish/English is desirable.
Experience: Two years’ experience providing services to highly vulnerable populations is preferred.
Education: Bachelor's degree from an accredited college or university in social work, human services or a related field.
Direct Reports: This position has no direct supervisory responsibilities.
Computer Skills: Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and widely supported internet browsers.
Certificates, Licenses and Registrations: Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
Valid automobile liability insurance.
Travel Requirements: Travel to meet with clients as needed.
Travel to meetings at multiple St.
Joseph Center sites and at community partner locations as needed.
