HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within the boundaries of the County’s eight Service Planning Areas (SPAs).
The County-wide Benefits Entitlement Services Team (CBEST) is a program under DHS’ HFH and provides targeted benefit advocacy services to assist individuals, who are homeless or at risk of being homeless (e.
g.
, individuals, families, children, Veterans, etc.
) who have complex health and/or behavior health conditions, high utilizers of public services (e.
g.
, Los Angeles County Departments of Mental Health, Public Social Services, Children and Family Services, Military and Veteran Affairs, Sheriff; Los Angeles Homeless Services Authority (LAHSA), in obtaining sustainable income through government programs such as Supplemental Security Income (SSI), or Social Security Disability Insurance (SSDI), Cash Assistance Program for Immigrants (CAPI), and/or Veteran Disability benefits.
POSITION OVERVIEW The Staff Analyst, Health position (also referred to as “position”) will manage, oversee, and directly supervise the following CBEST’s team (also referred to as “team”) as below: Outreach Team I: Department of Mental Health and Department of Health Services Team Such team is responsible for establishing and maintaining relationships with referral partners across re-designed target areas to coordinate the receiving/processing referrals.
The position is a part of the HFH/CBEST management team and will report to CBEST/HFH executive leadership personnel (also referred to as “HFH personnel”).
The position will require frequent travel to all eight Los Angeles County Service Planning Areas to meet with and engage service providers, community-based organizations, stakeholders and partners, and other healthcare, mental health, public social services, etc.
leaders/staff, attend various meetings with HFH personnel and/or HFH community/contracted agencies, and/or attend/participate (or present, speak, etc.
) at off-site HFH related meetings, conferences, etc.

Outreach Team I (Department of Mental Health and DHS Team) This team will be responsible for establishing/maintaining relationships with referral partners (e.

) across the re-designed target areas to coordinate, support, and maximize the receiving and processing of referrals.
This team will also facilitate presentations/trainings on HFH criteria and referral eligibility to increase CBEST client referral volume.
Additionally, this team may also assist with identifying clients eligible for Housing and Disability Advocacy Program (HDAP).
Duties and responsibilities include, but are not limited to, the following: Supervises, organizes, guides, and coordinates the activities, and workload, of HFH contractor personnel assisting HFH clients and/or HFH program personnel; assigns, reviews, and corrects work prepared by HFH contractor personnel accordingly.
Provides guidance to HFH contractor personnel in the appropriate application of policies, procedures, protocols specific to HFH-funded contracts, programs, etc.
Provides on-going technical assistance on complex issues to staff, partners, and contract agencies; assists in the analysis of problems and identification of solutions; assists in project modifications as needed to achieve project goals and objectives.
Coordinates ongoing enhancements for clients to ensure services have been provided and the clients’ cases are moving forward in effective manner.
Interviews potential HFH contractor personnel; makes recommendations for selection; provides applicable orientation and trainings.
Performs specialized duties requiring a working knowledge of specialized subject matter involving the use of initiative and judgment with procedural and policy limits.
Develops and prepares an array of correspondence, reports, materials, and spreadsheets for HFH personnel as needed.
Provides data entry/input, compilation, data retrieval, and analysis on various HFH databases, programs, spreadsheets, systems, etc.
Searches HFH records and files for data in which a required and/or sampling of information, data, etc.
is needed in selecting or abstracting HFH material, documents, data, client information, etc.
May assist with contract monitoring activities including annual administrative and programmatic monitoring; adheres to federal, state, county, and city policies and guidelines and complies with accomplishing any assigned task/s on the job accordingly.

Certificates/Licenses/Clearances A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related functions.
Successful clearance of a live scan process with the County of Los Angeles.
