As a Per Diem Registered Nurse, Care Coordinator at Cedars-Sinai you will be scheduled to work a minimum of 4 shifts per month on weekends.
Responsibilities include the following.
You will serve as a clinical co-pilot—charting the best trajectory of each patient’s care by accommodating various needs, including medical, psycho-social, financial and practical.

You will coordinate the care and service of the patient population from admission through discharge.
You will assess and Plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.
Educational Requirements:
BSN preferred.
Associates Degree in Nursing required, CA License required
Licenses:
Experience and certification in Case Management preferred
Case Management certification expected within 2 years of hire
BLS Certified
Experience:
3 years minimum in Acute Care Nursing
