*Job Title: *HDM Coordinator - Hollywood *Reports to*: Nutrition Director *Department: *Nutrition *Classification*: Non exempt *JOB DESCRIPTION* *Position Overview:* HDM Coordinator is responsible for overall coordination and implantation of Home Delivered Meals Program (HDM).
Supervises all drivers, Title V enrollees, and volunteers.
Coordinates and assigns routes for efficient meal delivery to clients.
Responsible for ensuring proper delivery of meals from caterer to HMPC on a daily basis, responsible for data entry of new clients’ information, update and printing of new route delivery sheets.
Ensures proper vehicle maintenance and up keeping.
Schedules and conducts in-home assessments for HDM participants to determine eligibility for HDM as well as for referral for other services.
*Essential Duties and Responsibilities:* * Coordinate routes, drivers and runners ensuring timely delivery of meals to clients.
Communicates effectively with caterer regarding meals quality, delivery and ordering issues and other relevant issues as needed.
Includes proper completion of Meal Delivery Ticket with correct temperatures and Daily Meal Ordering form.
* Responsible for data entry on new client’s information, update and print new route delivery sheets.
* Coordinates and oversees the work of drivers, volunteers and all staff assigned to delivery of meals.
Responsible for training new staff and volunteers.
* Coordinates the initial intake and home assessment of individuals requesting HDM to determine eligibility for service in accordance with Program guidelines.
Includes initial phone assessment prior to start of meal service and in-home assessment within 10 days after start of service.
* Provide relevant information and appropriate referrals for clients as needed.
Refer to Case Manager when client situation requires.
* Compiles statistics and maintains records relating to vehicle maintenance and utilization.
Conducts vehicle checks on a regular basis and arrange for repairs as needed.
* Supervises collection of donations from participants following HMPC fiscal procedures, assuring client confidentiality.
* Compiles and maintains accurate records and statistics for monthly reporting for funding sources.
Includes weekly reports and monthly reports to City Nutrition Agency.
* Provides feedback to Nutrition Director regarding any Program issues.
* Provides back up for HDM drivers as needed.
* Participates in trainings and staff meeting as requires by HMPC policy.
*Job Qualifications:* * Applicant must be fully vaccinated for COVID-19 as this is a requirement of all agencies who subcontractor with the City of Los Angeles.
Proof of vaccination will be requested and verified prior to employment.
Applicants may have a religious exemption and still be considered.
* Some specialized training in the field of aging or significant experience working with senior populations.
* Experience in making home visits to clients preferred.
* Supervisory experience preferred.
* Ability to work independently.
* Excellent interpersonal skills as well as the ability to work as part of a team.
Must enjoy working with older individuals.
* Must have the ability of extensive analytical, interpersonal communication skills.
* Must possess a demonstrated ability to work effectively with highly developed planning, organization, administrative and multi-tasking skills.
*Supervisory Responsibility* This position will have no direct supervisory responsibilities, may supervise Driver, volunteers or Title V workers as needed.
*Work Environment* This job operates in a professional Senior Center.
This role routinely works in the dining room and kitchen, will work with office equipment and computers.
*Physical Demands* While performing the responsibilities of this position, the employee is required to talk and hear.
The employee is required to stand, walk, climb steps daily, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl, have full range of motion, and the ability to lift 25 lbs.
Vision abilities required by this job include close vision.
While performing this job the individual performing these responsibilities may need to lift or move items or people weighing over 25 pounds.
Will pass required physical exam for Driving on annual basis.
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
*Travel* Daily Driving is expected for this position, in company supplied vehicle and backup driver as needed.
*Required Education and Experience* * High school graduate or equivalent * Food handlers certificate helpful * Clean DVM driving record * Valid CA DL *Language Skills: * Ability to read and interpret documents such as safety rules, operating and maintenance instructions, procedure manuals, ability to speak effectively before small groups of clients.
*Mathematical Skills: * No major mathematical skills needed, basic calculate figures and amounts.
*Reasoning Ability*: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form, ability to deal with problems involving facilities and building emergencies.
